I, the undersigned, hereby give my consent for Tiahni Housley to perform makeup services for me. I understand that these services will include a consultation prior to the appointment and the application of various makeup products to achieve my desired look.
I understand that the makeup application may involve the use of products that could potentially cause allergic reactions or other risks. I agree to inform the makeup artist of any known allergies or sensitivities prior to the service. By proceeding with the application, I release the makeup artist from any liability for any allergic reactions or adverse effects that may occur. I understand that while every effort will be made to ensure a safe and satisfactory experience, unforeseen circumstances may arise.
I acknowledge that in order to avoid a cancellation fee, I must give at least 48 hours' notice if I need to cancel or reschedule my appointment. In case of an emergency, and upon providing appropriate documentation, the makeup artist may choose to waive the fee at their discretion.
Furthermore, I grant permission for Tiahni Housley to take photographs of my makeup for the purpose of portfolio creation and promotional use, including but not limited to social media, websites, and marketing materials. I understand that my identity may be associated with these images unless I explicitly request otherwise.
